AGREEMENT FORM
My daycare is open from 6AM to 5:30PM, but I will try to work with your schedules to fit in the daycare’s timeframe. If you drop off your child late, you cannot pick up your child late. Remember, there is a late fee of $1 PER MINUTE. Be aware that the late-fees must be paid in full the same day serviced. For any reason you cannot pickup your child/children, please let me know who will pick up your child/children and he/she will need to show me their legal picture ID at the time of pick up. Also, there are no application fees. There is a required two-weeks of security deposit. My paydays are Monday mornings and you MUST pay at drop off. Somehow if your child is absent during the week when they’re scheduled to be here, the payment must be made in full, no excuses. Once a year, I get one full week paid vacation, five sick days, and two personal holidays. I’m closed on the following holidays: New Year’s Day, Memorial Day, Easter Monday, Independence Day, Labor Day, Thanksgiving, and Black Friday, Christmas Eve and Christmas Day. You will have to give me a two-week legal notice before leaving my daycare permanently. Additionally, if I see any legal problems, I will be terminating the child from my care before the two weeks. Finally, I will need a signed transportation permission slip.
PRICING
Pricing will be discussed during the interview where I work with parents (and their schedules) and the number of hours that I will take care of their kid(s).
HEALTH & SAFETY
• If your child is sick, please DO NOT bring him/her at my daycare. Also, if they get sick at my care, I will call you to come and pick up your child for the other children’s safety. If you provide your child any kind of medication the night before, I must be notified of what it is even if it’s Tylenol or Motrin.
• If your child’s temperature is above 100℉, the parent will be called to pick up their children. If you cannot pick up your child, please have a guardian pick up your child. He/she must be listed on my emergency form and have him/her bring in their picture ID at pick up time. This is for everyone’s safety.
• In order to bring your child back into my care, please make sure the child is fever-free for at least 24 hours. If not, a doctor’s note is required.
• If your child has medication, it must be from a doctor. Please make sure to bring in a doctor’s prescription including the doctor’s note, dosage, medication name/type, the original container, and the container must have the child’s name on it.
• As for safety, this daycare includes: safety latches on drawers/cabinets, smoke detectors, safety outlet plug-ins, gates for the stairs, evacuation plans, and fire drills.
• We also do not have small toys/objects for the children to choke on.
• Also, all sharp/pointy supplies are kept in a safe place away from the children.
INCLUSION STATEMENT
In my daycare, we do not discriminate against race, religion, personal background, ethnicity, or special needs/help.
FAMILY CONFERENCES
I will schedule at least two conferences a year to discuss your child’s progress that he/she gained from my care. I will be sending out a two-weeks notice before the conference with the time and date. The appointments may be scheduled to your convenience. You will be required to sign the slip and return it to me. I will also notify you through a text reminder two days before the scheduled conference.
GENERAL SCHEDULE
• In the morning, children will be served breakfast around 8AM.
• After breakfast, they will have one hour of play time.
I will be helping the children during the play time with slightly challenging games such as puzzles, checkers, etc. Then, we’ll study together, learn how to write, read the alphabet, know the colors, and how to count. This will happen not only with plain reading and writing, but also with books, puzzles, cards, and many more.
• After that, the children will be served snacks.
• If it’s nice outside, we will go outside for nature walks, collecting leaves and/or rocks for art projects, and have some outdoor play time.
• When we get back inside, we will continue with more studious activities.
• Then, lunch will be served.
The children will also clean up after themselves for every snack and meal they’re served.
• After lunch, the children will take a nap for as long as they feel.
• Once the children wake up from their naps, they will use the bathroom to freshen up*.
• They will then eat snacks once more.
• When that’s over, the children will be able to have some free time to play with other children or do other activities such as coloring, drawing, reading books, writing their names and/or numbers.
After any activities, the children will help clean up.
*Infants are fed and changed as needed.
Please note that this is a general schedule and activities change according to the season and weather, like going to the park is common during spring and summer whereas indoor playing is common during the fall and winter.
DISCIPLINE POLICIES
• If two children are fighting over the same toy, I will direct one of them to another toy so the other one can play with the first toy. Then they will switch so everyone has a chance to play with the toy. Additionally, I will provide choices such as letting the children play with toys, coloring, or other activities he/she likes to do.
• If the children don’t follow the instructions or listen, then they will be in time out for a few minutes.
• If one child pushes or fights another child, then I will make them apologize to each other, let them sit separately for a couple minutes, and inform the parents what happened. After that, I will make an official note of the incident.
• Children who respond back with an unacceptable attitude will be put to time-out and the guardian shall be notified.
